Adapter plate is attached to Manfrotto head in just a few simple steps:

Remove original Manfrotto platform with provided hex wrench.
Attach conversion plate in place of original Manfrotto platform using original screw.
Attach clamp to adapter plate using provided screws.

Like the previous reviewer I like my Manfrotto 393 large lens head but have grown tired of switching plates because all my other mounting hardware is Arca-Swiss compatible. And the new-used 500mm lens has an Arca-Swiss compatible replacement foot.
This conversion set is excellently manufactured, easy to install - less than 5 minutes - and obviously strong. The large diameter knob is easy to find and tighten and the large diameter allows a good grip so you know the clamp is tight.
Do not let the photos on the web site fool you the clamp and plate are bulkier (a good thing) than they look and I have great confidence the set is up to the task.
The quality is impressive enough that I ordered conversion sets for my Manfrotto 400 and 229 heads heads also.

I love my Manfrotto 393 Gimbal head, but I also use another gimbal/tripod combination that takes Arca-Swiss plates. It was cumbersome to keep changing plates in the field, depending on which tripod I was using. This adapter eliminates that problem once in for all.

It is very well made and comes with all the wrenches and mounting screws you will need. You have to read the directions on the website to install it because none are included. Otherwise, it would have been a 5 star rating.
